Adult Craniopharyngiomas£ºSurgical Results with a Special Focus on Endocrinological Outcomes and Recurrence according to Pituitary Stalk Preservation

Craniopharyngiomas originate in the hypothalamic and pituitary regions, which are of paramount importance in endocrinological regulation. It is generally believed that surgery for craniopharyngioma results in a high rate of hypothalamopituitary dysfunction. However, only a few consecutive series have demonstrated the actual frequency of endocrine deficiency associated with surgery for craniopharyngiomas. This review focuses on the surgical treatment of craniopharyngiomas, with a special focus on the endocrinological outcomes and tumor recurrence in cases of the pituitary preservation. We reviewed 47 surgical cases of adult craniopharyngioma in our institute. Despite the preservation of the stalk at surgery, 33.4% of the patients showed normal hormone function, and 8.3% of the patients received partial hormone replacement postoperatively. However, the pituitary stalk must be preserved with maximal tumor resection whenever possible in order to increase the chance of intact anterior pituitary functions being maintained. The results of the present study show that pituitary stalk preservation may not be related to increased recurrence rates.
